The Power of Attorney is needed to allow Law Label Services to sign and submit the registration request forms on your behalf.

The Power of Attorney ONLY gives Law Label Services the ability to SIGN YOUR APPLICATION on your behalf. It does NOT give any other rights to Law Label Services!

Without the POA, the forms would have to be sent to you for YOU to sign and then you would have to mail them back where they could be lost or delayed.

You only need to do this once.

Step 1: Download the POA Here

Step 2: Print and Sign the forms (with witnesses)
  1. Print the POA on your companies letterhead paper
  2. Foreign companies should stamp the document with company stamp
  3. Domestic companies should have it notarized

Step 3: Email to info@lawlabel.com OR fax to (001) 614.543.8348

Step 4: LLS will keep it on file to use in the future.
